Abstract
U ovom radu pozornost je posvećana logičkom oblikovanju baza podataka, kao jednom od najvažnijih koraka oblikovanja, i to prvenstveno primjenom normalnih formi. No, prije samog logičkog oblikovanja potrebno je dobro definirati korisničke zahtjeve za sustav koji gradimo, kako bismo mogli što lakše odrediti sve entitete i njihove atribute koje moramo uključiti u naš model, a kasnije eventualno i u bazu podataka. Svi prikupljeni korisnički zahtjevi popraćeni su i potrebnim dokumentima iz domene vođenja medicinske dokumentacije prilikom hospitalizacije pacijenta, te prikazati i pripadajućim UML dijagramom slučajeva korištenja.
