Abstract

Najčešća posljedica moždanog udara je jezični poremećaj koji se naziva afazija. Kod afazija se radi o narušenosti već stečenih sposobnosti što može biti vrlo frustrirajuće i za one koje pogodi i za njihovu okolinu. Ostavlja značajne posljedice na kvalitetu života, budući da se negativni utjecaj na komunikaciju i jezik proteže i na socijalni, emocionalni, profesionalni i funkcionalni status pojedinca, pa čak i na mentalno zdravlje. S obzirom da se radi o naglim i velikim promjenama, nije čudo da članovi obitelji, osobe emocionalno vezane za osobu s afazijom, teško prihvaćaju ove promjene, teško se snalaze i prilagođavaju. Tu je vrlo važna uloga logopeda koji će objasniti na koji način je komunikacija oštećena, što se može, a što ne očekivati od osobe, kako tumačiti određena ponašanja, na koji način pristupati osobi i slično. U nekim su područjima kvalitete života posljedice afazije itekako vidljive, iako se istražuju u znatno manjoj mjeri, kao na primjer područje utjecaja afazije na članove obitelji. Posljedično tome, naše je prepoznavanje i razumijevanje potreba osoba s afazijom i njihovih članova obitelji oskudno.
Cilj ovog rada je dati pregled značajne i stručne literature, odnosno, istraživanja u kojima je naglasak na kvaliteti života osoba s afazijom i njihovih obitelji. Svrha ovog rada je, temeljem dosadašnjih istraživanja, steći uvid u učinke afazije na kvalitetu života onih njome pogođenima te steći uvid u promjene u komunikacijskim obrascima, kao i u njihov učinak na socio-emocionalni život obitelji. Nadalje promiče se pomak ka uključivanjem procjene kvalitete života u standardnu kliničku praksu logopeda kako bi se ciljevi postavljali u odnosu na stvarne potrebe osobe. Također, u radu je posebno naglašena i prepoznata važnost uključenja obitelji u rehabilitaciju te su prikazani različiti terapijski pristupi koji su usmjereni na komunikacijske, konverzacijske i druge potrebe osobe s afazijom.

Abstract (english)

The most common consequence of a stroke is a language disorder called aphasia. Aphasia is an impairment of already acquired abilities, which can be frustrating for those affected and their surroundings. Aphasia leaves significant consequences on the quality of life as its negative impact on communication and language extends to the social, emotional, professional and functional status of the individual, and even to mental health. Given that these are sudden and major changes, it is no wonder that family members who are emotionally attached to a person with aphasia find it difficult to accept these changes and find it difficult to cope and adapt. A very important role here plays a speech and language therapist who can explain how communication is damaged, what can and cannot be expected from a person with aphasia, how to interpret certain behaviours, how to approach a person with aphasia and so on. The consequences of aphasia are very visible in some areas of quality of life, although they are being researched to a much lesser extent. One of those areas is the impact of aphasia on family members and consequently, our identification and understanding of the needs of people with aphasia and their family members is scarce. The goal of this paper is to provide an overview of significant and scientific literature and research in which the emphasis is on the quality of life of people with aphasia and their families. The purpose of this paper is, based on previous research, to gain insight into the effects of aphasia on the quality of life and changes in communication patterns of those affected, as well as their effect on the socio-emotional life in affected families. Furthermore, a shift towards incorporating quality of life assessments into standard speech and language therapist clinical practice is being promoted in order to set goals in relation to individuals’ actual needs. This paper also emphasizes and recognizes the importance of family involvement in rehabilitation, and presents various therapeutic approaches that focus on communication, conversational and other needs of people with aphasia.
